Want to complete the form ONLINE?
Some forms, but not all, can be filled out online after downloading. An asterisk * beside a form number/title indicates that after the form has been downloaded to your computer, it can be completed by clicking in each field and typing into the field if desired. Please be aware that unless you are using a FULL version of Acrobat (not just the free Acrobat Reader), a form completed online can be printed, but the DATA contained in the form is not saved. Entering data is for ease of data entry and appearance. Please print and make copies of any forms you complete in this manner before quitting the form. All data is lost once the document is closed. All forms can be used in the usual manner (fill out by hand, typewriter, etc.) simply by printing the form.
